CoP 5: Interventions for Integrated Management of Chronic Conditions

October 21, 2025 – February 10, 2026
(Every other Tuesday, with a two-week holiday break)
2:00 p.m. to 3:00 p.m. ET

This interactive, eight-session Community of Practice (CoP) will support medical, behavioral health, and clinical and non-clinical support staff at HRSA-supported health centers to develop a more integrated care approach to treating patients with chronic conditions.


CoP 6: Expanding Behavioral Health Services Integration in Health Centers

October 22, 2025 – February 11, 2026
(Every other Wednesday, with a two-week holiday break)
2:00 p.m. to 3:00 p.m. ET

This interactive, eight-session Community of Practice (CoP) will show HRSA-supported health centers how to implement and sustain effective behavioral health service expansion in their care settings.


CoP 4: Integrating Addiction Treatment Services Into Primary Care

October 23, 2025 – February 5, 2026
(Every other Thursday, with a two-week holiday break)
1:00 p.m. to 2:00 p.m. ET

This interactive, eight-session Community of Practice (CoP) will guide HRSA-supported health center staff to integrate SUD services into their primary care setting.
